CHS Eligibility

This is to inform Quileute Tribal Members and other Native Americans what is requried to be Contract Health Service Eligible (CHS Eligible).

To be eligible for Quileute CHS, Native Americans must be:

1. Enrolled Quileute and:
   1. Have been living within the "CHSDA" as described below, or
   2. Be a full time post-secondary school student (qualifies for EXTENSION of services, not initiation).
2. Enrolled, Non-Quileute Native American, but have been living on Quileute Reservation OR working for the Quileute Tribe at least one year (continuously).

Notes:

1. The Quileute CHSDA (Contract Heath Service Delivery Area) consits of Clallam County.
2. For Quileute Tribal Members:
   1. Quileute CHS eligibility expires 180 days after a Quileute individual moves out of the CHSDA (except for extension for full time students of post-secondary school programs).
   2. If inactive, Quileute CHS eligibility resumes (or begins) 180 days after a Quileute individual moves back into the CHSDA.
3. For Tribal Members of other Federally Recognized Tribes:
   1. Quileute CHS eligibility terminates immediately when a non-Quileute individual moves off the Quileute reservation, and will not resume until after the individual has returned and lived on the Quileute Reservation for one contiguous year.
4. All Tribal Members that are CHS eligible MUST:
   1. Apply for Alternate Resources such as Medicaid, Medicare, or Private Insurance.
   2. If the Tribal Member does not qualify for Medicaid, Medicare, or Private Insurance, they must apply for Basic Health Insurance. This is an Insurance Sponsor Program offered to Quileute CHS eligible Native Americans that is provided by the Quileute Tribe.
